Successful Transitions
Reentry to Resilience (R2R) helps youth successfully transition from a Youth Development Center (YDC) back into their community. From the day we get a youth referral, we begin to develop a customized approach to ensure success from the day of release and for 12 months post-release. Working with families, communities, schools and employers we:
- Reduce recidivism or repeat offenses
- Increase academic success
- Enroll students in college
- Increase independent living skills
- Engage students in the workforce
- Reengage with families and the community
A Youth Success Coach (YSC) actively works with each individual youth, his/her Juvenile Court Counselor, social worker, educators, and family. And once the youth returns home, we immediately:
- Manage school enrollment and provide academic support – tutoring, learning labs, etc.
- Connect youth’s to afterschool and pro-social support programs – sports, service learning, mentoring, etc.
- Prepare for career and workforce readiness – vocational education placement, mock interviews, job shadowing, job placement, etc.
A proven systematic approach for successful transition
Using our model of wraparound community supports and services, we follow a proven systematic approach to ensure we attain individual educational, family and career goals for each youth. A three phase approach with planned check-in’s one year after release allows the Youth Success Coach to mitigate potential issues and to provide encouragement and direction to realize optimal outcomes.
Phase 1: First 30 days of referral
- The Youth Success Coach (YSC) engages the Juvenile Court Counselor, educators, family, and the youth in need
- Set up the monthly Service Planning Meeting (SPM)
- Gain a complete understanding of the overall education goals, career readiness factors, workforce entry potential and vocational education needs
- Create strategies and a plan for academic and work-life success.
Phase 2: Three to six months prior to release
- Youth Success Coaches develop a Transition Intervention Plan (TIP) to detail education and academic goals after release
- Coordinate education enrollment, grade placement (Base school, alternative school, or virtual school), GED completion, vocational school enrollment and/or community college enrollment
- Balance educational needs with career readiness activities to provide support for accomplishing graduation and post-graduation educational goals
- Assist with professional certification and placement in the workforce
- A comprehensive plan customized to meet educational needs and career potential
- Connect with necessary community resources
- Work closely each youth at every step of their educational growth and career path
Phase 3: Post-Release support up to 12 months
- Youth Success Coach creates a plan for the first 30 days of release
- Identify all critical education components and addresses all workforce opportunities
- Manage school enrollment, review class schedule and, if needed, coordinate transportation
- Assign tutors and mentors and sign-up in afterschool and recreational activities
- Conduct a career interest inventory, provide job shadowing and 1-on-1 job coaching
- Engage the family support system in the process as much as possible
- Monitor goal progress with school staff and family to maximize student success
