Location: Rocky Mount, NC
Reports to: Director, Youth Development
The Youth Success Coach will case manage a multi-phased support structure for youth returning to the home, school, and community from a Youth Development Center. The YSC will participate in pre-release planning with the YDC transition team, which will develop a support and intervention plan that will follow the youth while in the YDC and after transition out of placement. Prior to release, the YSC will work with families and schools to prepare a successful reentry plan / strategy. YSC will work primarily in the Youth Development Center with staff and families, attending service planning meetings for each student at least once per month. Post-release, the YSC will work with other YSCs to transition them back to their home community. In addition, the YSC will support the youth and family’s therapeutic services as recommended by the risk/needs assessment, as facilitated by Juvenile Court Counselors.
Specifically, the YSC will encourage academic, physical, and social-emotional growth; discourage involvement in gang or anti-social activities and offer school-based opportunities for engagement in pro-social/positive activities that promote cognitive and behavioral change in all settings.
- Attend monthly service planning meetings for each student assigned at the Youth Development Center
- Coordinate pre- and post-release support plan and probationary requirements with Juvenile Court Counselors.
- Identify students’ pre-release at the Youth Development Center (YDC) and utilize provided assessments to determine strengths/deficits in education, social, emotional, familial, physical to develop post-release support plan.
- Provide pre-release coordination, planning of resources for the needs of youth.
- Counsel, advise & monitor students around academics, school attendance, behavior and post-secondary opportunities.
- Link school team professionals, parents, along with appropriate agency/community personnel for the purpose of addressing adjudicated student issues.
- Coordinate recognition for all adjudicated students who show improvement.
- If appropriate, connect student and parent to the appropriate CIS Student Support Specialist at the school for services and continued supportive case management.
- Assist parents in post-release process – through ensuring that previously committed students returning to a CIS school site will be enrolled and provided case management.
- Align with other community based agencies that deliver services to siblings/families of incarcerated youth/adults.
- Complete appropriate documentation to capture services provided to students, as well as other necessary documents.
- Maintain electronic caseload notes and necessary attendance, behavior, and coursework metrics for progress, monitoring, and adjusting interventions.
- Work with students post-release to support their transition back to the community on an as needed basis
- Frequent sitting, speaking, hearing, moving about the school and/or Youth Development Center
- Occasional lifting (20 lbs.)/ Occasional pushing (25 lbs.)
- Travel Required: Daily travel 75% and occasional overnight travel.
- Bachelor’s degree (Master’s Degree preferred) in the field of education, human services, social work, counseling, criminal justice, public administration or management related field.
- A minimum of 3 years of working experience in one of these areas.
- Preferred experience working with the adjudicated population.
- Strong interpersonal skills
- Self-motivated with the ability to make independent decisions
- Sensitivity and ability to appreciate points of view of personnel within the schools, YDCs and other agencies/organizations, including a keen awareness of their needs and goals and how to build collaborative partnerships based upon fundamental CIS principles.
- Ability to acquire and maintain familiarity with all relevant sources of resources
- Ability to be flexible in a work environment often filled with ambiguous situations
- Effective oral and written communication skills
- Professional presentation skills with both large and small groups
- Computer skills to include working knowledge of Microsoft Office software
- Ability to manage time, paperwork and people effectively
- A valid NC driver’s license
- Knowledge of youth behavioral issues/community programs and resources available
- Persons willing to develop supportive network of services for identified youthful offender
- Bilingual (English/Spanish)
